On June 19, 2013, James died unexpectedly at the age of 51, just days before he was scheduled to travel to Sicily to accept an award at the Taormina Film Festival. James had spent the day sightseeing with his family in the sweltering heat.

Following that, his son Michael discovered him unconscious on the bathroom floor of the Boscolo Exedra Hotel at approximately 10:00 p.m. local time.

Then he contacted the hotel’s receptionist, who contacted emergency paramedics. James arrived at the hospital at 10:40 p.m. He was pronounced dead twenty minutes later.

An autopsy confirmed that he died of a heart attack. As word of his death spread, politicians such as John McCain and New Jersey Governor Chris Christie took to social media to express their condolences.

On June 24, Governor Chris Christie of New Jersey directed that all state buildings in the state fly flags at half staff in honor of Gandolfini’s return to the United States.

Gandolfini’s body was returned to the United States on June 23. Michael expressed gratitude to both the Italian and American governments for expediting the repatriation process, which typically takes seven days. On June 26, in honor of Gandolfini, Broadway theater marquee lights were dimmed.

Gandolfini’s funeral service took place on June 27, 2013, at the Episcopal Cathedral of St John the Divine in Morningside Heights, Manhattan. His body was cremated and the cremated remains were returned to his family.
