Ottavino was drafted 30th overall by the St. Louis Cardinals in the first round of the 2006 MLB Draft out of Northeastern University. That year, he pitched for the Swing of the Quad Cities of the Class A Midwest League.

He pitched for the Springfield Cardinals of the Class AA Texas League in 2008. Ottavino pitched for the Memphis Redbirds in the Class AAA Pacific Coast League in 2009. (PCL). He had a 0–9 win-loss record in the first half of the season, but a 7–3 record in the second half.

Ottavino has developed into a well-rounded pitcher capable of pitching in a variety of late-inning situations. Ottavino also holds the Northeastern record for most strikeouts in a career with 290.
